He is survived by: Gina Houx-Kotarski, Mother, of Dolores, CO; Marcus Kotarski, Brother, of Westminster, CO; JT (Jeremiah) Kotarski, Twin Brother (Alina Umberger), of Dolores, CO; Dr. Mary Leiker, Grandmother, of Perth, Australia; F. Ron Kotarski, Grandfather, of Cortez CO; Zach Aman, Uncle, of Perth Australia; Sam Houx, Uncle, of Dolores CO. He leaves behind many beloved cousins, family, and friends.
A Celebration of Jake's Life will be on Saturday, October 7, 2023 at 4pm at the Dolores Fire Department in Dolores, Colorado.
Preceded in death by Felix Ron Kotarski, Father, of Dolores Colorado; MaryAnn Houx, Grandmother, of Dolores Colorado; and Melvin Ray Houx, Grandfather, of Dolores.
